About ACO REACH

CMS introduced ACO REACH (Realizing Equity, Access, and Community Health), an alternative payment method to empower primary care physicians and eligible professionals to move beyond fee-for-service compensation and into the realm of value-based payments.

ACO REACH provides the following benefits to reduce common value-based care frustrations:

  • Simplifies patient attribution
  • Focuses on chronic medical conditions and encourages follow-up visits
  • Establishes a network of participating Preferred Providers
  • Offers financial stability through a fixed per-member-per-month payment model
  • Reduces expensive and challenging quality reporting obligations
  • Promotes health equity to underserved communities

Welcome to Florence Health

At Florence Health, we are committed to revolutionizing the healthcare industry. Our namesake, Florence Nightingale, was a symbol of unwavering dedication in the field of healthcare, and it is with that same commitment that we forge ahead in our mission to create a primary care system the world deserves. We started with a focus on older adults in the United States because they are the “power users” of health care and each ounce of improvement in care delivery can impact their quality of life.

Join us on this mission and become a part of the enhanced future of healthcare.
